Super fund documents & forms

Forms for individuals

Below are the forms you will need to become a member of Future Super, as well as forms regarding rollovers, withdrawals, insurance and other transactions with the fund.

Note: the quickest and easiest way to become a member of Future Super, and rollover from your old super fund at the same time, is to fill out the online Application Form.

  • Application Form: Complete this form to become a member of the Future Super fund. Alternatively, you can download the printable Application Form.
  • General Compliance Certificate: Employers will require this certificate to set up contributions into your Future Super fund.
  • Rollover Form: Complete this form if you are already a member of the Future Super fund, and would like to rollover some money from another super fund into your Future Super account.
  • Standard Choice Form: Employers may require this form to be completed when an employee chooses to join Future Super.
  • Personal Contribution Form: Complete this form if you would like to make a personal contribution of money into your Future Super account. If you are making a self-employed concessional contribution (before-tax), you must also complete the Notice of intent to claim or vary a deduction for personal super contributions form and send it to us with your Personal Contribution Form.
  • Life Insurance Application Form: Complete this form if you would like to access death, total permanent disablement, or income protection insurance through your Future Super account. 
  • Life Insurance Transfer Application Form: Complete this form if you would like to transfer your current insurance coverage from another fund to your Future Super account.  
  • Non-Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Future Super account in the case of your death.
  • Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Future Super account in the case of your death.
  • Withdrawal Form: Complete this form if you would like to apply to withdraw all or some of your super money.
  • Early Release Due To Financial Hardship Form: Complete this form if you would like to access all or some of your super money due to financial hardship. To access funds for severe financial hardship, you will also need to complete a Future Super Withdrawal Form.
  • Early Release Due to Permanent Incapacity: Complete this form if you would like to access all or some of your super money as you are unable to work due to a medical condition.
  • Adviser Services Form: Complete this form if you have a financial adviser and you would like them to have access to your account information. You can also complete this form if you would like an Adviser Service Fee paid from your super account to your financial adviser.
  • Change of Details Form: Complete this form to update your personal details with Future Super.
  • Contribution Splitting Form: Complete this form to split your superannuation contributions.
  • Work test declaration: Complete this form if you are over the age of 65 and would like to make a personal contribution into your Future Super account. Please contact Future Super with further questions regarding the work test and how it applies to you.

Forms for financial advisors

  • Adviser Dealer Group registration: Complete this form to register your dealer group with Future Super in order to be paid an adviser services fee through your client’s account.

Below are the important documents potential members should read before making a decision to join.

  • Product Disclosure Statement: Contains all the important information needed to make an informed decision about joining the Future Super Pension Plan.
  • Financial Services Guide: Contains important information about who we are and your rights as a client.
  • Significant Event Notice: Contains important information about an upcoming change to the structure of the fund.
  • Significant Event Notice: Contains important information about the closure of Transition to Retirement Pension for members under 65.
  • Significant Event Notice: Contains important information about the closure of Transition to Retirement Pension for members over 65.

Forms

Below are the forms you will need to become a member of the Future Super Pension Plan, as well as forms regarding withdrawals, beneficiaries, advisers and other transactions relating to your account.

Note: the quickest and easiest way to become a member of the Future Super Pension Plan is to fill out the online Application Form.

  • Application Form: The quickest and easiest way to become a member of the Future Super Pension Plan (and rollover from your existing super fund at the same time) is to fill out the online Application Form. Alternatively, you can download the printable printable application form.
  • Rollover Form: You can make a transfer from another super fund before you commence taking income payments from your Future Super Pension Plan. The best way to do this is by providing your rollover details on the online pension application form. You can also do this by completing the Rollover Form. Once you are receiving an income from your Future Super Pension Plan, you are unable to make additional rollovers to your account.
  • Personal Contribution Form: You can make a personal contribution before you commence taking income payments from your Future Super Pension Plan. Once you are receiving an income from your Future Super Pension Plan, you are unable to make additional contributions to your account. If you are making a self-employed concessional contribution (before-tax), you must also complete the Notice of intent to claim or vary a deduction for personal super contributions form and send it to us with your Personal Contribution Form.
  • Pension Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Future Super account in the case of your death.
  • Withdrawal Form: This is the form to complete when you would like to make a lump sum withdrawal from your Future Super Pension Plan.
  • Adviser Services Form: Complete this form if you have a financial adviser and would like them to have access to the details of your Future Super Pension Plan. You can also use this Form to make an Adviser Service Fee payment to your financial adviser from the balance of your Future Super Pension Plan.
  • Reversionary Form: Use this form to nominate reversionary beneficiaries to your account.
  • Change of Details Form: Use this form to update your details with us. Alternatively, email your change of details to info@myfuturesuper.com.au.
